A woman from Guatemala developed a bifrontal headache and fever. After sleeping for 24 hours, she was difficult to arouse, somnolent, confused, and unable answer questions or follow commands, with fever to 38.3°C (101.0°F). The pupils reacted sluggishly; extra-ocular movements were grossly intact. The white cell count was 17,900 per cubic millimeter (neutrophils 14,770 per cubic millimeter).
