- History of Present Illness
2016 IDWeek Fellows' Day Case Presentation: A man in his sixties developed blurred vision in his left eye. On the third day of his symptoms, he presented for evaluation.
He reported having had a flu-like illness 2 months earlier, which was followed one week later with the onset of intermittent fevers, chills, night sweats, fatigue and almost daily mild headaches. He had also noted early satiety, decreased appetite, and a 20-pound weight loss. One week before this presentation, his hands and feet became swollen, and he developed occasional tingling and burning sensations. He had no joint pain, skin rashes or painful skin nodules.
- Past Medical History
- He was in good health before the flu-like illness two months earlier.
- Medications
- He took acetaminophen and naproxen as needed for headache and fevers.
- Social History
He lived with his wife and children in Southern California and had worked as a landscaper until this illness, when he became unable to work due to his symptoms.
- Epidemiological History
He was born on an island in the South Pacific and immigrated to the United States many years earlier. He had no recent international or national travel, no sick contacts and no known exposure to tuberculosis. He owned several dogs, with no other recent exposure to animals. He had smoked marijuana daily but stopped one month before this presentation. He had no history of tobacco, alcohol or intravenous drug use.
