A middle-aged woman had a 6-day history of fatigue, anorexia, fevers, chills, severe headache and myalgias, in late summer. She had recently hiked in New England and had a dog; she was not sexually active. The temperature was 103.5°F (39.7°C), blood pressure 104/50 mm Hg, and pulse 122 beats per minute; the remainder of the exam were normal. The peripheral blood smear is shown.
